A root cause analysis (RCA) is the process of finding the root cause of a problem and exploring different solutions to fix the problem. A root cause analysis helps individuals and teams to systematically think through a specific problem to properly identify sustainable solutions to prevent the problem from happening again in the future.

This flowchart aims to guide individuals and teams in root cause analysis and has the following structure:

Event
Problem description
Gather data
Identify impact
Determine causes
Identify corrective actions
Plan for mistake proofing
Implement and followup
Determining if the solution is acceptable or not

After participants have filled out the sheet, a group discussion is encouraged to synthesize key learnings and determine priority action steps.
