The Patient Pathway Visual Toolkit

The Patient Pathway Visual Toolkit is practical tool to help patient organisations, advocates and engagement professionals map real-world care experiences, uncover challenges, and plan meaningful advocacy actions. Designed as part of the Patient Pathway Toolkit, this board supports co-creation, strategic thinking, and stakeholder alignment.

We encourage patient involvement stakeholders to use this tool to identify and prioritise the challenges patients experience and be able to build strong advocacy objectives for their respective communities.

The Patient Pathway Toolkit is a co-created resource developed by Patvocates, hosted by the European Patient Advocacy Institute, and funded by Boehringer Ingelheim. It is available under the Creative Commons Attribution-ShareAlike 4.0 International Licence (CC BY-SA 4.0). Use of logos and branding elements is excluded from this licence.

✅ Step-by-Step Usage Instructions

Note: This template is designed for use alongside the full Patient Pathway Toolkit, available at https://www.patientadvocacy.eu/key-tools-and-resources/. For more detailed guidance, refer to the “Facilitator’s Guide” included in the toolkit.

  1. Duplicate this template to your own Miro workspace from the Miroverse page.

  2. Review the "Guide" panel on the left side of the board to understand the purpose of each section and how to use the board effectively.

  3. Map the ideal care experience by using sticky notes to fill in each stage of the pathway (Prevention & Screening, Diagnosis, Treatment, Supportive Care & Quality of Life) from the patient’s perspective.

  4. Identify challenges by adding sticky notes or comments beneath each relevant step.

  5. Capture potential actions that patient organisations and other patient involvement stakeholders can take to address each challenge.

  6. Tag key stakeholders responsible for each action using the colour-coded icons available on the board.

  7. Prioritise next steps by moving the most urgent or feasible actions into the “High Priorities” section.
